Primary care

Ongoing care for adults: yearly checkups, new symptoms, medicine refills, screenings and referrals, with a provider who gets to know you.

What it is

Primary care is your main point of contact for your health. Your provider listens to what is going on, checks how you are doing and helps with everyday concerns such as colds, back pain, skin rashes, headaches or trouble sleeping.

It is also where prevention happens. We help you keep up with screenings, vaccines and blood tests, refill the medicines you already take and refer you to a specialist when you need one.

What to expect at the visit

  1. You check in at the front desk or from your phone. A nurse or medical assistant takes your blood pressure, pulse, weight and temperature.

  2. Your provider asks about your symptoms, your health history and anything else on your mind. There is time for questions.

  3. If an exam or a test would help, your provider explains it first and asks for your agreement.

  4. Before you leave, you get a plain summary of the plan: prescriptions, tests, referrals and when to come back.

  5. Your visit summary appears in the patient portal within one business day.

How to prepare

  • Bring a photo ID and your insurance card, if you have one.
  • Bring every medicine, vitamin and supplement you take, in the bottles if you can, or a written list.
  • Write down your top two or three questions so they do not get forgotten.
  • If you are new to us, finish the online registration form at least one day before your visit so we can start on time.
  • Wear something easy to roll up or take off, in case your provider needs to examine your arm, shoulder or knee.

Questions about primary care

Can I see the same provider every time?

Yes. When you book, you can choose a specific provider. We encourage it, because a provider who knows you can notice small changes over time. If your provider is away, any other provider on the team can see you and has your notes.

Do you refill prescriptions?

Yes, for medicines you already take, once your provider has reviewed them. Ask for refills through the patient portal or by calling us at least three business days before you run out. Some medicines need a visit first.

How do referrals to a specialist work?

Your provider decides with you whether a specialist would help. We send the referral and your records for you. If your insurance plan needs approval first, our team handles that request and tells you what to expect.

Can I talk about more than one concern in a visit?

Yes. Please tell the nurse at the start, so your provider can plan the time. If there is a lot to cover, your provider may ask you to come back for a second visit rather than rush.

Prefer to talk to a person?

Our front desk can book, change or cancel an appointment, answer questions about your visit and set up a time with an interpreter.
