Chronic condition management

Regular visits and a clear plan for long-term conditions such as diabetes, high blood pressure, asthma, thyroid problems and high cholesterol.

What it is

A chronic condition is a health problem that lasts a long time, such as type 2 diabetes, high blood pressure, asthma, thyroid problems, high cholesterol or arthritis. Good care is steady and planned, so small problems get fixed before they become big ones.

Together with your provider you set clear goals, such as a blood pressure number or a way to feel steadier day to day. We check progress at regular visits, adjust medicines when needed and connect you with our dietitian or counselor when that would help.

What to expect at the visit

  1. At each visit, a nurse checks the numbers that matter for your condition, such as blood pressure, weight or breathing.

  2. Your provider reviews how you have been, what is working and what is hard.

  3. You agree on any changes to medicines, food or activity, and the next set of goals.

  4. Tests such as blood work are ordered at the right time, with the reason explained.

  5. You leave with a short written plan and a date for your next check-in. Some check-ins can be done by phone.

How to prepare

  • Bring all your medicines, and any home readings such as blood pressure or blood sugar.
  • Bring your meter, inhaler or other device if you use one.
  • Write down any side effects, missed doses or trouble getting your medicine.
  • Bring recent test results from other clinics or hospitals if you have them.
  • Think about one thing that would make your day-to-day life easier.

Questions about chronic condition management

How often will I need to come in?

It depends on the condition and how steady you are. Many people come every three months. When things are settled, it can be less often. Your provider will set the schedule with you.

Can some visits be done by phone?

Yes. When your numbers are steady, some check-ins can be done by phone, with readings you take at home. Your provider tells you which visits can be by phone.

Do you manage more than one condition at once?

Yes. Many people have two or three, and treating them together often works better. Tell your provider about everything, even if it seems unrelated.

What if I cannot afford my medicine?

Tell us. Our team can look for lower cost options, ask about patient assistance programs and talk through your choices. Please do not stop taking a medicine without speaking to us first.

Prefer to talk to a person?

Our front desk can book, change or cancel an appointment, answer questions about your visit and set up a time with an interpreter.
