Alder Health

Your first visit, step by step

Everything to do before you come in, the forms to bring, how the patient portal works and what to expect from our policies.

Before your first visit

  1. Book your visit online or by phone. You get a reference and a calendar file.

  2. Download and fill in the three forms below, or fill them in at the front desk. If you do, arrive 15 minutes early.

  3. Bring a photo ID and your insurance card. No insurance? Bring nothing extra, and see our self-pay prices.

  4. Bring every medicine, vitamin and supplement you take, in their bottles or as a list.

  5. Bring records from other clinics if you have them, and write down your questions.

  6. Tell us about any accommodation you need, such as an interpreter, a quiet room or extra time.

  7. If the patient is under 18, a parent or legal guardian comes to the first visit.

Forms to print

The same questions are on paper at the front desk. If a PDF is hard for you to use, call us and we will read the questions to you or send them in plain text.

The patient portal

After your first visit we email you an invitation to a secure portal. You choose your own password, and you can ask us to add a family member as a proxy.

Alder Health is a fictional clinic, so there is no live portal on this website.

  • See visit summaries, test results and your medicine list.
  • Send a secure message to your care team and get a reply within one business day.
  • Ask for a medicine refill and choose your pharmacy.
  • Pay a bill, update your insurance and change your contact details.
